Requesting Medical Records
To obtain medical records for services provided by Nemours or to receive copies of an X-ray, please print and complete the Authorization to Release Form. You can return the form by:

- The processing time for copies of records is 8-10 business days.
- For personal copies of records, a flat rate of $6.50 will be assessed (CD, email, fax or mail).
- Please call (866) 956-7299, press option #1, to check the status of your request.

If you have observed conduct that appears to violate a Nemours policy or a legal or ethical standard, report the concern to the Compliance Helpline. It is managed by a third-party to protect confidentiality, and is available 24/7. You can choose to identify yourself or remain anonymous.
